Tearing Visions. Perceiving a splinter’s form causes immense pain. If a creature starts its turn within 40ft of the Splintered and can see it, it must make a DC 12 saving throw taking 2d8 slashing damage on a failed save or half as much damage on a successful one.

Unstable Form. If a splintered takes 10 or more damage from an attack, it must use its reaction causing reality to react violently around the attacker. The creature must make a DC 12 dex saving throw. Taking 2d8 force damage on a failed save or half as much damage on a successful one.

Actions

Jagged Touch. Melee Weapon Attack: +5 to hit, reach 5ft, one target. Hit: (3D6+2) slashing damage.

Splintered look like pale and twisted humanoids with thin jagged spikes splayed out from their bodies in random directions. These spikes seem to shift position and length as the creature or observers move, not unlike images in a broken mirror. Their eyeless faces show horrified or grimacing expressions and often their limbs are bent at odd and painful angles. As they shamble about aimlessly, they produce a shrill sound of scratching glass.

Splintered have lost all sense of self, and wander without purpose. They are attracted to living beings, as though they’re a reminder of a previous life. Regardless of intent, this is extremely dangerous for other creatures as the splintered tears at their form and senses.

Knowledge Checks

Knowledge (Arcana) DC 15 - The creature is unstable and partially extraplanar in nature.

Insight DC 10 - The creature causes only harm, there is no reasoning with it.

Treasure

When a Splintered dies its form collapses in on itself. At the site of collapse there is some residual extraplanar dust that continues to stay stuck between planes. There is enough of the substance to carefully fill 1D4 small vials.

Vial of Phase Shift

Wonderous Item, Uncommon
This small vial contains extraplanar dust. By throwing and shattering the vial at their feet, a user may cast the spell Misty Step. After this use the vial and its contents are destroyed.
